Rx ACE’s mission is to foster healthy communities through education, outreach, prevention, and collaboration. We focus on the wellness of communities with access to less resources. 

Our program works to reduce stigma around substance use disorders and address the risks of prescription medication misuse—while promoting acceptance and supporting for the needs of affected individuals and communities.

Why Naloxone Matters

Naloxone saves lives. It’s a safe, easy-to-use medication that can reverse the effects of an opioid overdose. Whether you’re a parent, peer, or provider—knowing how to use naloxone can make all the difference.

Know the Signs of an Opioid Overdose:

  • Unresponsive to sound or touch
  • Vomiting or gurgling sounds (“death rattle”)
  • Slow, irregular, or no breathing
  • Bluish or gray skin tone
  • Pinpoint pupils
  • Irregular or stopped heartbeat

Freedom and Hope (Esperanza Libre)

Empowering Communities Through Equity, Healing, and Support

At Carolinas CARE Partnership, the Freedom and Hope (Esperanza Libre) program is committed to advancing opportunities for individuals who have historically had access to less resources, including people impacted by incarceration, substance use, homelessness, poverty, and systemic barriers to care.

Grounded in harm reduction, resilience-oriented care, and peer empowerment, our program provides free, flexible, and person-centered support to help individuals reclaim their health, dignity, and future.

Who We Serve

The Freedom and Hope (Esperanza Libre) program is open to all individuals who meet the following criteria:

  • 18 years of age or older
  • Resident of Mecklenburg County

Our services are especially designed to support individuals who may be navigating:

  • Reentry after incarceration
  • Recovery from substance use or mental health challenges
  • Housing instability or homelessness
  • Economic hardship or limited access to care
  • Social or systemic barriers to wellness and support

We are committed to providing resilience-oriented care that respects the dignity and lived experiences of every participant.

Peer-led support (individual and group sessions, virtual or in-person)
Reentry navigation and second-chance support
Mental health and substance use recovery resources

Housing and community resource referrals
Culturally responsive outreach and advocacy

Our Approach

We believe in meeting people where they are with empathy, respect, and a commitment to self-determination. Our team includes peers with lived experience who understand the challenges of navigating systems and are here to walk alongside you.

Funded by the Charlotte-Mecklenburg Opioid Settlement (CMOS), Freedom and Hope is more than a program; it’s a pathway to healing, connection, and long-term wellness.
